Customers of health and social care services should have significant or full control of their lives, including a home of their own they are happy with, where appropriate adaptations have been made to ensure they can live independently and receive support where and when it is needed. The housing partnership is responsible for this, and continues with the following activities:

  • HEART service – Housing Adaptations and the Disabled Facilities Grant.
  • Strategic thinking about the use of home adaptations and assistive technology offer to support people to maintain their independence at home and strengthen their resilience.
  • Delivery of statutory and discretionary housing functions designed to improve the quality of people’s home environment.
  • Manage the expenditure against the Disabled Facilities Grant.

Reviewing the Housing Related Support Contract in preparation for re-tendering / commissioning in 2021.
