Customers of health and social care services should have significant or full control of their lives, including a home of their own they are happy with, where appropriate adaptations have been made to ensure they can live independently and receive support where and when it is needed. The housing partnership is responsible for this, and will achieve its outcomes as follows:

  • Prevention and early intervention housing related support to enable people to remain in their homes or make suitable housing choices before the point of crisis.
  • Development of an integrated approach to housing, social care and health, taking into account the housing needs of Warwickshire’s most vulnerable and using data and intelligence production and validation to target care where it is most needed.
  • Strategic use of home adaptations and assistive technology to enable people to retain and maximise their independence for as long as possible. Expenditure will be managed against the Disabled Facilities Grant, and services delivered through the Home Environment Assessment and Response Team will be developed.
