Limb restriction care services re-launched

Local NHS providers and Warwickshire County Council, through the Warwickshire Cares Better Together programme, are working together to help people with limb restrictions across Warwickshire receive care in the most appropriate setting.

Two new services have been launched:

  • Short-term (time limited) support to return home, for people with upper limb restrictions such as a cast (Plaster of Paris) or brace. This pathway commenced on Monday 22 July and will cease on 21st January 2020;
  • Care home beds commissioned for people with lower limb restriction, that are non-weight bearing, for a period of time and unsafe to return home directly from hospital. This service will be delivered in South Warwickshire only, and 3 beds will be facilitated at Stratford Bentley NH between Monday 9th September and  8th March 2020. 

Any healthcare professional based in the acute or community hospital can refer to these services. Each acute provider has been sent a separate communication with the referral method, consent and patient information in order to cascade to relevant colleagues within their organisation.

At South Warwickshire NHS Foundation Trust, the complex discharge team will complete referrals at the acute site for the upper limb pathway.
