ConnectWELL signposting for specialist services

ConnectWELL provides local residents with the opportunity to improve their health and wellbeing.

This could be through various leisure, health or welfare activities and services.

As a signposting provision, they provide expertise on pointing users in the direction of the specialist service they require.

They provide information on support for debt and housing, to sport, art and craft and self-help services.

Should a user be anxious about attending, they have a Health Buddy service also available. A member of the team will attend the group or activity with the user, and over a period of time withdraw support to encourage independence.

ConnectWELL is a Social Prescribing service based in Rugby and Nuneaton, and developing capacity to extend further into North Warwickshire.

Being the first and only Social Prescribing project of its kind in Warwickshire, it is a resource GPs and other Health Care Professionals are backing.

Their service is for those who live in Rugby and Nuneaton and are over the age of 18. Under certain exceptions they can work with families of mixed ages.
