Three new hospital step down services commissioned
South Warwickshire NHS Foundation Trust and Warwickshire County Council are working together to commission a number of hospital step down services to help people across Warwickshire get care they need outside an acute setting.
Thanks to additional winter funding the two organisations have been able to introduce three new services that will support people who do not medically need to be in hospital to get the care they need in a more appropriate setting.
Night support service
It includes a short term night sitting service facilitated by domiciliary care agencies.
The main aim of this service is to provide encouragement and support so people regain or retain their independence, and to support them to remain at home with confidence.
Referrals to this service will be by any relevant professional who identifies the service as appropriate for the customer. This can include the Community Emergency Response Team, Hospital Social Care Teams, Community Social Care Teams, Discharge Coordinators, George Eliot Hospital Navigators, and Reablement Team.
Domiciliary Home Care Support for upper limb plaster of Paris and / or braces
The aim of this service is to provide customers with support in their own home for a period of 6 to 12 weeks pending assessment. It is expected that these customers will require the support of two carers (double ups) due to their restricted movement and for safe moving and handling.

Reduced mobility care home service
This is a care homes based ‘Reduced Mobility’ step down pathway for individuals deemed as unable to return to their original place of residence due to temporary non weight bearing ability caused by unstable fractures or plaster of Paris on their lower limb.
Referrals to this service will be by designated Discharge Coordinators or Therapy Teams based within the following acute hospitals:
- Warwick Hospital
- George Eliot Hospital
- Rugby St Cross
The ‘Reduced Mobility’ pathway is a short term service. The aim is to provide support whilst the plaster of Paris is on in a care home for a period of 6 to 12 weeks pending assessment.
